Mixed skins aren't welcomed here. HP bar came from the Black 2014 skin. I'm not sure which skin the mod icons came from though, but I recognize them. There are probably several other elements that came from other skins.

what's wrong if i bump this thread? 
The thread is months old, meaning that the creator has probably abandoned it, otherwise it would've been bumped with an update by now.
