Hi TimeGhost, As requested, here is a general mod of your Insane and Hard. There are still some fundamental issues I have with your mapset, and i've discussed them at the end of each difficulty.

00:08:575 (10) - Why not continue the pattern of triplets you had going on in 00:06:975 (1,2,3,4,5,6,7) - ?
00:13:375 (1,2,3) - Evenly space.
00:37:175 (7,8,9,10) - Evenly space.
00:46:175 (5,6) - ^
02:08:575 (1,2,3,4,5,1,2,3,4,1,2,3,4,1,2,3,4) - Really cramped. Consider another pattern that is more readable.
02:13:375 (1,2,3,4,5,6,7,8) - Make them symmetrical. 7, 8, are noticeably different.
02:19:775 (11) - Make slider symmetrical.
02:33:775 (1,2,3,4,5,6,7,8,9,10,11,12,13,14,15,16,17) - There's no reason to space out the stream.
02:37:175 - You should have the slider here.
Rhythmically it's not bad, but this difficulty has almost no patterns or symmetry etc... I understand you hand-mapped a lot of this difficulty, which is a problem, because it leaves the difficulty rather messy. Incorporate more patterns instead of just clicking and placing a note somewhere. Think about why you are placing the circle or slider there, and think about why they are in that particular shape/form. If you don't know why, then it's best to think up a pattern first.
There are also spacing issues throughout the map. You don't have to use distance-snap, but you should still space your circles/sliders evenly so that the map is more tidy. Uneven spacing also contributes to bad flow.

00:35:775 (2,3,4) - 1/4 sliders aren't necessary on Hard. You don't even use them on Insane

02:13:375 (4,5,6,7,8,9,10,11,12) - Improve the pattern. Make it even.
02:23:375 (2,3,4) - 1/4 sliders again.
02:34:175 (4,5,6,7,8,9,1,2,3) - Streams again on Hard.
You use a lot of unnecessary jumps in your Hard, as well as streams. Consider making them easier... Same with Insane, this difficulty needs more patterns and structure. The spacing needs to be improved. Many notes are hand-mapped and thus look messy. Typically Hard maps will use at least a bit of distance-snapping. Some mappers even use DS for the majority of their Hard difficulty, only breaking it for jumps.
Going through your Easy and Normal quickly, I can say they are much better because of their distance snap. They are neater. There are also more blankets. However, they still lack in patterns imo.
This is just a general mod, I don't want to weigh you down with all the minor issues (imperfect stacking, blankets, not parallel, minor NC changes, flow, etc...) what I've listed here are more important. I trust you'll use them to improve your map. Look at other maps in editor and see the patterns they use in their maps. Maybe they'll give you some ideas on what to put in yours? The best way to get better at mapping is to mod others' maps.
