just go to file > save as new difficulty
if for example, you're making a new 4k difficulty from a 7k map, just rename it to "4K MX/HD/NM/whatever" in the general tab, then go to the difficulty tab and change the column count

if you mean what you said about NM/HD/MX, they're just ordered by difficulty as EZ (easy) NM (normal) HD (hard) MX (insane)
those are the standard names, i guess, you can name them anything as long as it makes sense
