Where I can find this life bar?

I already have a skin what is basically elements of other skins (because i really suck at skinning) and I can´t find this life bar, there is a image what shows the life bar

I want it for my skin, but with my name, I saw that life bar on Heibel videos too, but with his name instead Cookiezi one.

I don´t know in what skin i can find this bar (because there are like 500 Heibel skins and only one of Cookiezi but there is not the life bar in that skin) and how to put my name in it, please help.

Name change pack is there as well. All you do is use something like Photoshop and change your name. Done
