How can I improve at reading? Also confusion tendency

I tend to get confused alot, pressing D when I have to press K.
I mostly play muzukashii maps, getting an A if lucky, but doing 85-90% on most maps.
A friend told me that playing easier maps with HR could help me.
Tips?

Honestly this is a problem I still have even now where I'll occasionally read something as a don and hit it as a kat or vice versa.

Best advice I can give you is that this is often related to a specific kind of pattern or just losing concentration during slower parts. Your best bet is to practice the specific patterns that are causing you to mix up your hands and to play maps with higher SV (which is where HR is nice) to get used to reacting quicker and comprehending notes faster.
