3 Months ago I was able to sightread most 7* maps and was on par with a lot of 5 digits in MP. I decided to take a break for a week or two and came back to suddenly realize I'm getting worse. It's been a month now as I try to relearn and get better but all I see are the same results. Nothing is progressing and my motivation to climb/play is slowly deteriorating.

TL;DR
I use to be decent at the game, now I'm terrible. How do I get better?

Im becoming more inconsistent as time passes and it just gets frustrating to realize I'm worsening. I can barely play 6*+ maps anymore and sometimes would fail simple 4-5* maps.

What really messes with my head would be that I used to be able to stream 220-240 comfortably but now I can barely stream 180.

Cuz you took a break. You have to de-rust, which can take time, and certainly won't happen in a day or two.

Alternatively, maybe you're playing maps that are too hard for your actual skill level and/or using mods that are making maps that would be doable too hard for you regardless of that break, and because you've lost the ability to just keep up with them that lack of skill is showing through.

Either way, don't try to play what you used to right off the bat. Start with easy stuff that you can do but with a bit of difficulty, then start working your way back up to where you were. Most importantly, don't be impatient with yourself. Realize that getting back in the groove isn't an instantaneous thing; like with riding a bike you can still remember how to, but that doesn't mean you can just hop onto the nearest mid-high-difficulty mountain bike trail your first day back. Keep at it, and you'll return to where you were.

in those situations, it's important to take a step back on 7* maps since you can no longer sightread those

when you take a break, you have to relearn the hardest maps that you used to play before, which means derusting and all

as Voidedosu said, start playing low stars maps, then try to climb the ladder until you reach the level that you had before taking your break
