Nothing much to add except hit sounds:

Insane:
01:08:31 (7,8,9) – You could reduce the volume of this section by one tick or so, so that it matches the fading out effect of the music. Then make 01:21:19 (1) louder so it has more of an effect.
01:19:72 (9,10,11) - ^

Nothing else I can suggest, star.

1:02:02: Try maximizing the audio in this timing section and see if you like the effect more
01:03:54 (1) - Maximum audio timing section here. I feel the full effect is kinda lost at this point where the audio is still 2/3. The audio can be turned back down after this beat though.
01:28:60 (1) - Where this is placed... I think it'd be a lot better if the rest of the vocals were mapped and the slider was made to begin at 01:30:05 and end at 01:35:75.

01:09:06 (3,1) - it fits the lyrics better if the new combo is on 3 instead of 1

01:28:624 (1) -
You could move this spinner to 1:30:050 to add more beats. It sounds too early for a spinner IMO.
Also you can add a finish here. It really sounds better. (If you find it too loud, then maybe soften the volume at the end of the spinner.)

Insane
01:10:725 (6) - Sounds weird, IMO
01:28:624 (1) - Same as above on Easy
