Does it mean not playing the game for over 30 days or last seen over 30 days ago in osu? I'm so confused.

yeah pretty much. It happens after a month or so.

I mean, is it last seen 30+ days ago or not playing 30+ days ago?

Pretty sure it's the former. I haven't played taiko in years but my country ranking there isn't greyed out.

pretty sure it means "not logged in" because there's someone hasn't played for months and isn't greyed out

Yeah that's what I mean. You can try testing it by comparing user's last seen to the greyed out name's ones.

except if someone has "hide our online presence" options turned on

IIRC if you went offline 30+ days, your name is greyed out. Possibly seen on maps' leaderboard, and also global/country score/pp leaderboard.

But, if there's a pp rework, and you're not playing for 3+ months, you're REMOVED from the leaderboard.
Yes, disappear, not grey out. But it will go back once you start being active again.

There's probably a big enough sample size to make a reasonable conclusion.
