The ranking criteria says: For difficulties above the lowest required difficulty level, the spread cannot skip any difficulty levels and there cannot be any drastically large difficulty gaps between any two difficulties

Is a jump from Normal to Insane "drastically large", or is it rankable? (2.20 to 4.72, map drain time is 2:45)

you’ll need a hard difficulty in between a normal and insane

a 1.5* gap in difficulty is too big for me but it depends

You can't skip from Normal to Insane, that's already spelled out by the "cannot skip any difficulty levels" part. You need a Hard unless your drain time is over 4:15 in length.

________

When every difficulty is mapped, a drastically large gap would be something like a very easy Normal going to a very challenging Hard. This is more detailed depending on the maps and modders may recommend adding an additional difficulty in-between if the gap is too big

Think for example something like going from a Normal that barely uses 1/2 to a Hard with constant 1/4 triples and some jumps. They are their respective difficulty levels but spread wise are very far apart and would probably need to be adjusted.
