I think it's a .png file so, you'll have to convert it to a .ico file.
The rest you may need to Google for instructions unless you already know how.

With the effects?

Windows cursor wouldn't support the effect (cursor trail) as you can't specify a custom trail image.
It also wouldn't support the 'clicking' effect.

The basics of it is:
#1: Upload your cursor to here in order to convert from PNG to ICO.
#2: Then drop the converted ICO image into C:\Windows\Cursors
#3: Then go to 'Control Panel' > 'Mouse' then click the 'Pointers' tab.
#4: Select 'Normal Select' from the 'Customize' field, click Browse and replace it with the ICO image.

I'm not sure whether there's an option in Windows to change the point origin.

On the 'Pointer Options' tab you'll find 'Display pointer trails', but I don't think that'll produce the effect you're after.
However, It wouldn't be too difficult to create an application which can simulate the same effect (with a user defined trail image).

They are saying it's not possible to have the mouse trails and all the other things the cursor does in game.
The most you'll have is the cursor looking like the in-game cursor with no effects.
