Right, so first of all there is something really important: If you pick a pattern, stick with it. Don't keep changing the rhythms or hitsound times. Go with the flow. It makes it sound a lot better and it's better for play.
Next: I think what Bakyugan means when he/she says change "Avert Fate" to Insane is that you should keep the names consistant. This kind of ties into what I said above. Either you want all of your Beatmap Difficulty names themed or just named normally.

I'll start with a pattern issue.
[0:07:292] (2) Alright, here you've started a pattern - a Finish hitsound every two beats. So either remove it or stick with it. I'd prefer if you stuck with it, but it means you should add another at (0:09:031), (0:09:901), and so on until you reach (0:14:249), where another pattern starts. Really complete your patterns.
There are a lot of errors in combos - I'll try and squish a few.

Next, with the new slider selected, go to the "Edit" toolbar option at the very top of the window, then select "Flip Horizontally." And there's your new slider.

[0:33:814] (1) Finish histound and the 1 and the end, clap hitsound on the reverse arrow.
[0:36:423] (2) With the pattern you made, you should remove the finish hitsound and add a clap.

Sorry, I got a bit tired during the Avert Fate map - that isn't a bad thing, it's just I probably missed a lot of stuff.
ANYWHO, I think I'd move this back to WIP for just a bit longer - I knwo you really do think it's good enough to be ranked, but there are a lot of technical issues. I probably pointed out quite a few, but there are probably more. Anyway, move it to WIP, there are usually more modders over there too, and tell me what you think.
