This just happened today recently when I was testing my map in play mode. I was testing out the skin that in progress for my project map and then I noticed the fail-background.png I made for it refuses to work.
Here is the screen shot of it as well: Screen Shot 1 (This is when the fail-background "flashes in" and then fades out.) / Screen Shot 2 (This is after it fades out and then left with the default blank black background.)
And its not just my map that has this problem. Even Queen - Don't Stop Me Now has the same problem (along with other maps having the issue as well).
