Problem details:
I have an issue when I'm playing that theres a small chance (Like 5-10%) that my keyboard will register my single tap as a double tap and it causes me to fail a lot when two circles are on top of each other. This has happened with two of my mechanical keyboards and my sayo device. I will link a video below so you can see that i am only single tapping

EDIT:
I found the issue. It turns out that My fingers were actually sliding too close to each other and clicking both keys at the same time. My way of solving this is by changingg my keys to be on two different rows instead of next to each other. So instead of Z and X, I am now using W and D. This should eliminate the issue since the keys would not be able to be pressed at the same time
EDIT 2:
Another way I found to fix this issue is by rotating my keyboard. Since my right finger was the one that overlapped and pressed both keys, i rotated the left side of my keyboard up (if your left finger overlaps, push the right side of your keyboard up). So far this is a better fix for me since having keys on different rows made it harder to stream and finger control became harder

It turns out that I am still having this issue. I decided to use the program "Rewind" to show my replays and when I am "clicking" the keys

In this image, the orange, gray, and green dots show when the timing window is and when I am holding down on the keys. The orange bar is my left key [Z], the green bar is my right key [x], and the gray bar is the timing window for a circle. This circle was standalone and was not in a burst or stream. As you can see, I clicked down on my left key but my right key was also pressed down right after the circle occurred. Below I will link more screenshots of this happening on the exact same playthrough of the map on both sliders and circles

As mentioned before, this happens on MULTIPLE KEYBOARDS. I have deleted and reinstalled OSU several times, yet this only happens on OSU. It has happened on Stable and Cutting Edge but I have no tried Lazer yet (I will try it after this edit and update the post accordingly).

This also happens on the Lazer and Beta releases of OSU. I have made a tech support post on the OSU Discord and am currently discussing this issue
Edit 5 (Final edit, issue solved):
I finally figured out how the issue was happening. It was key ghosting, and it wasnt my finger sliding over and hitting both keys. But it turns out that when I wanted to click just one key, both of my fingers would push down and both keys would press, but the second key press was so subtle it only lasted for a few frames. Listed below is a handcam video of this issue

I fixed this issue (finally) but switching to tapping with my index and ring finger, as opposed to my ring and middle finger. So far this issue has almost ceased to exist, save for a few instances here and there thought I just have to get used to the new tapping style first
