In my case its moment I see a triple, I instantly know it's a triple and well... 3 alternate taps and I'm done.
You can see is it a triple or double or stream/whatever based on how many circles you just... see there. I guess how well you do them is somewhat based on how well you can react. It can be hard based on the bpm+AR combo but the more you play different types of maps, the more you'll know about getting used to those.

Perfect stacks are harder than normal stacks, especially with hidden. Some skins have slightly transparent circles which makes it kinda easier I guess... Just play and you'll eventually learn how to read them.
Umm like... First use a skin that have a slight transparency, so you can discern the numbers of each circles. I use this one: https://circle-people.com/wp-content/Skins/Cookiezi/Cookiezi%2017%202016-02-27.osk (although i changed the cursor and spinner of mine). The next thing to do is to learn to discern the number of each circle. Try to count how many circles start to pop out in each stack. If you see three circles, tap thrice. If you see 5, tap 5 and so on.
You could also first play the no hd variant of the map (no mod/dt only/hr only) to learn the map. It may help with reading with hd in the long run, but i can't say this for sure since i haven't tested this (i play mostly no mod)
