Not solely b/c they requested it. If there was overwhelming evidence that long-term survival would be slim and/or they would never fully recover to (at least moderately) comfortable lifestyle, than I might consider it after consulting their friends/family/other doctors.

I think he mean't the way he characterizes himself is diverted from the norm to which others would perceive him as strange.

A: My ability to articulate good conversations is horrible. I always say something weird or incorrectly and never know what to say to keep it going.
