Because you're using circles in your default-x elements they'll show two circles as soon as the combo is two digit. The 3rd circle in the middle is the circle of hitcircle/overlay.png. Makes sense?
Use the default-x elements for your numbers (or make them all transparent in case you don't want numbers) and it should be fine. If you use numbers or something different than transparency for it, you may have to adjust the value of HitCircleOverlap in your skin.ini.

so to fix it go to ypur skin.ini file and find

HitCirclePrefix:
HitCircleOverlap:

If they are not there then put them under [Fonts]

HitCirclePrefix: default (probably you understand that there should be)
HitCircleOverlap: 124124 (width of default-0)

i did it but now, 1-9 are fine, but 10+ are without "numbers" (in my skin it's these blue circle)

is all defaults the same?

it's funny how the same exact same problem appears on this forum multiple times, yet it's not mentioned in the FAQ

about the problem, all defaults need to have the exact dimensions
second, check the width of them. it is the first number that appears in dimensions (the one that is NOT @2x)
go to skin.ini, find HitCircleOverlap, set it to the number you found

strach all of that explanation, look at FAQ, the solution is exactly the same as the two dots problem
