Do maps sometimes get their difficulties manually reduced causing you to gain less PP on maps that you have already beaten?

Yes. Mods also get rebalanced.

If you used NF or EZ for example, you get less pp than the original you would get without the mods.

I think that's an answer for a different question than the one being asked.

If they were modified the leaderboards would reset
And you'd get pp according to the new difficulty
(Not sure though)
