This needs a lot of work. The map should match the music.

For instance:
- The first note longer than a single beat in the entire song is at 00:16:722, yet you have 4 of them before then which don't match to anything. Get rid of them. If you want a mixture of holds and hits, then make both match the music
- Notes should be where the music is. For instance the "hai" (which I think are what you are trying to map the 5 key presses to) actually start on the 7/8 beat before you have them (eg. at 00:05:681 rather than 00:05:722) - so either put a hit there or a hold starting there and ending at the same time as the singing(?) does - on the half beat eg. at 00:05:889. This is also the case with the "hai"s at around the 32 second point and onwards.
- Similarly, there shouldn't be a note where nothing happens in the music. There is a LOT of this in the later parts of the map, eg. at 00:28:139.
- More notes being pressed = more or stronger notes in the music. For example at 00:06:389 to 00:07:556 the stronger beat is the half beat, yet you've put double presses on the weaker notes on the white bars. You need to get rid of the long note anyway, so perhaps something like this would be more appropriate: http://osu.ppy.sh/ss/1012194
- Make the patterns match the map, if possible. For instance a staircase would fit at 00:38:056 but doesn't in the bit just after, which is where you've put one. Remember that you can have staircases of chords too.

Go through the whole thing for each of these suggestions in the order I posted them (you can do the second and third together if you want - in fact I recommend that you do), and this beatmap will be a lot better than it is currently. Good luck!

Other things:
The SV changes are almost nauseating and don't really serve any purpose. I would suggest getting rid of them. All. At the end of the day it's up to you, though.
Why does Kiai time stop for such a short period of time? Just keep it on from when it first starts to where it ends. IF you want to play around with SV, putting the changes at the points where kiai is currently turned off (and then back again where it is turned back on) would make more sense than what you currently have. I still suggest getting rid of them all, though. Obviously, Kiai time needs to be the same on all maps.
Set a preview point in all difficulties and make sure it's in exactly the same place for each one.
