This is the grip I use, I actually stopped playing with tablet for a while because I was getting a lot of pain in my thumb whilst playing. I recently decided I missed how much quicker I could be with a tablet and have started using it again.
The pain isn't as bad but maybe it's worth changing how I hold the pen though the thought of having to suck harder than I do at the moment until I get used it makes me sad.

The pen that came with this tablet is smaller than my older one (But a lot lighter!), so my pen grip changed slightly. Before my pen was gripped between my middle finger and ring finger, now it just rests on the tip of my middle finger and just on my ring finger's distal phalanx (where your finger nail is).
I must say though: after using a cheap tablet then converting to a high-end tablet, you really notice a difference on how far your pen can go from your tablet before disconnecting as well as pen weight.
