Whenever I lift off my fingers at the end of a slider, I always get a great or a "100".

You are either lifting too early, or too late. Osu combines both your press and the release into 1 timing judgement (at least in score v1). If you are too early or late at releasing the note, even a perfectly timed press will be reduced to a lower judgement like this.
use score v2 to see if the problem is at the start of the slider and not the release

try counting before the end of the long note to get used to where to lift off
something like "t minus 5, 4, 3, 2, 1, we have lift off"
