Many players just play easy/normal songs while on hard/insane diffs not much that's why your score goes sky high when you finish the song.

Why you sometimes get a D and a higher rank or an SS and a lower rank depends on the number of people who played the map. If only 1,000 people played the map, you'll get 1,000+ regardless of your grade (D, C, etc.) On the other hand, getting an SS and a low rank, if 30,000 people played the map, you could get much lower ranked than 1,000 (30,000+). I think that's how it is, anyone feel free to correct me if I messed it up.
