So this is not an issue but beatmap nominators dont follow their own rules weather for approval or not, alot of these rule are in the ranking critrtia page, which you can find here:
https://osu.ppy.sh/help/wiki/Ranking_Criteria

the ranking procedure can be found here:
https://osu.ppy.sh/help/wiki/Submission/Beatmap_Ranking_Procedure\

some of these rules include:
1.maps being longer then 5 minates ranked. the procedure states "Following up from ranked, a BN can approve these beatmaps if they are longer than 5 minutes long (aka marathon beatmaps) and/or that do not follow the ranking criteria but are considered "okay" by a BN"
2. audio files, i only saw this be the case once, but then again it was approved. even so the criteria states" A beatmapset may only contain one song file used by all difficulties. Multiple song files within a single beatmap set is unsupported and results in unexpected behaviour with preview times, metadata, etc" the map that i saw that did this was approved anyway so yeah.
3. this rule has been taken lightly so much but its the background image. the criteria states "You must have a background image on every difficulty of your beatmap. Different background files for different difficulties is acceptable."

thats all I have. I just thought id make you aware....

please correct me if im wrong

Most points you mentioned (from the page at https ://osu.ppy.sh/help/wiki/Submission/Beatmap_Ranking_Procedure) are outdated according to my knowledge. Nevertheless, the beatmap ranking procedure is not a part of the ranking criteria afaik.

The latest in effect ranking criteria could be found at https://osu.ppy.sh/help/wiki/Ranking_Criteria .

I could also give specific replies to the the three points you posted.

1. This is about spread requirements. The latest information could be found at https://osu.ppy.sh/help/wiki/Ranking_Criteria#Beatmapset You are possibly referring to an outdated rule.

2. Some old maps might have multiple mp3s because this rules might not be in effect then.

3. Didn't quite get what you mean, do you have any instances for this point?
