So i got a rank S on a 4 star map and i didn't got the medal, maybe because the map supposed to be above 4 stars because the map was exactly4 stars.

You have either dropped a slider end or Sometimes you don't get the achievement, I don't know why but just try to full-combo another 4 star, Next time you'll get it.
