General-- Make the kiai times consistent in all difficulties by ending the Normal's kiai time at 01:20:767 like the rest of the difficulties. It doesn't make much sense why it ends on that off-beat in the first place.

Insane00:28:125 (4) - I'm not sure if a repeat slider is what you want to do here; my main qualms against this slider is that of three reasons: first off, having the same slider that starts in one section and ends in the next section of a piece of music just doesn't play very well. Secondly, having a slider's end-point lie on a heavier beat than it's start-point is generally bad practice and should be avoided in most cases (however, it can and does work some of the time.. just not in this case). Finally, it messes up this map's combo placement. I would remove the repeat and add a hit circle at 00:28:691. You should also add a new combo.
00:33:219 (5) - Add a new combo here because the vocals have gotten louder and more dramatic... and it is a downbeat.
00:40:012 (1,2) - This section doesn't flow very well. Yes, it follows the vocals nicely, but I think adding a hit circle here would more effectively follow the music as a whole.
01:19:493 (6,1) - This pause, for the lack of a better term, is awkward. This is more of a light suggestion, but if you want a great way to transition out of this kiai time then I would make the hit circle at 01:19:493 into a 1-beat slider (which would accompany the vocals fantastically) and add a hit circle at 01:19:918. If you do follow this suggestion (you should!), add a new combo at the newly created hit circle that is right after the 1-beat slider you just added.

01:23:031 (8,1) - Switch the combos around here. The reason you should have a new combo starting at 01:23:031 (8) instead of the hit circle after it is because of that heavy beat in the background. Also, it's on a downbeat and you did the same thing at 01:25:295 and 01:27:559.
JBSparta's Hard00:28:691 (1,2) - Gah, so awkward. It doesn't follow the music well and it disturbed me while playing. You should definitely switch this around -- place the slider on the downbeat where (1) is and move (1) to 00:29:682. I think this follows the music a lot better so you should definitely try this out.
00:30:956 (1) - If you followed the suggestion above, shorten this slider by two ticks and add a hit circle at 00:31:946.
00:49:069 (4) - Start the new combo here and remove the one at 00:49:635. Combos are based off the music, not the patterns. It'd make a lot more sense to see a new combo at the start of a kiai anyways. While we're on this note, I'd like to point out that the pause between this hit circle and the next slider is unnecessary. You should make the hit circle into a 1-beat slider for more movement.
