It's a spinner glitch that is probably triggered by lag.
Happened to a friend of mine when he was recording osu!+Skype with Fraps.

It can also happen when you let auto play one of ph0x's binary songs. It confuses the shit out of the osu bot and causes it to spin at retard speeds.

Actually that extra spin happens when people are playing with spunout in multi.
Once time I played with people in multi and thing like that happened too.
