Hello , one of the problems are:

1) Delete the "mp4.video" and convert it in avi.
2) If not Works, delete video file in the folder.
3) download blank audio.mp3 and put in in the folder of the song. and update it, good luck!

Update!
I edited hard mode, it should be more manageble to play.
(The ending of it was at insane level difficulty at one point!)
I removed the opening video, and edited the name and author of the map.
P.S. Didn't intend to toss in normal and insane yet, they're incomplete lol.
I will have a future update on those as well, so please train on what i have completed so far or just ignore normal and insane for now.

Update!
I edited hard mode to make it more manageable to play (was 3.1 stars, now 3 stars)
The difficulty is a bit more evenly toned and notes do not seem too unreasonable.
I fixed the image from not appearing at all in edit / play mode.
