Problem Details: For the past 2 weeks or so, I have been seeing a lot of random slider misses and breaks even when a note is hit PERFECTLY, meaning, cursor is dead on center of the slider note, and my hit error doesn't leave the blue zone (as I like to call it), and I follow the slider perfectly, and yet I get a random slider miss, it usually displays 2 seconds after I got to next note/ slider and my PC specs are high enough to make osu perform with low input MS even when playing borderless with letterboxing, I can't provide a video or screenshot as fraps captures letterboxing as well and also locks my frame rate at 60 and screenshot is not possible as it happens when I play

Video or screenshot showing the problem: N/A

osu! version: 20160411.4 (latest)

Can you provide a replay file?

That, will be very difficult to do, as 1. I usually rage quit on that kind of stupid miss cause it's usually on a ranked map that I do not have a ranked score 2. Maps that I did submit that kind of play have lost their replays due to the fact I had to re-install osu! due to a corruption of it, all I can say is that people have been suggesting it might be the input delay which can be possible as I do play borederless, but I'm not gonna 100% say that it might be that, as I remember my VERY old play of Snow Drive (The unranked version that Cookiezi played) which I did for fun, and it had very late show misses on jumps that I thought I hit, it might be my PC just being old, or it might be a small visual issue

READ Carefully! "fraps captures letterboxing as well and also locks my frame rate at 60" when frame rate is locked at 60 osu get huge MS Spikes, so I can't record a Live play, also, "1. I usually rage quit on that kind of stupid miss cause it's usually on a ranked map that I do not have a ranked score " I currently refuse to submit choke plays as they tilt me to the point where I do not want to play osu and last time I had that kind of a wieard miss I threw my pen at my monitor and broke both the pen and the monitor (now I have a new pen and monitor) so... you know, I don't want to cause any more damage to my property that cost money and I worked hard for, also "2. Maps that I did submit that kind of play have lost their replays due to the fact I had to re-install osu! due to a corruption of it" PLEASE READ CAREFULLY what I say before asking for stuff I can't provide right at the snap of a finger!

I have no idea what to suggest unless I have a visual or at least some way to see what's going on.

I may have found a replay where I randomly break combo on a slider (322 combo is where I broke) and it does look to me like I hit the slider and followed it but I can hear the combo break

Beatmap: https://osu.ppy.sh/b/728001 (the play happened when the map was qualified)

Okay so I watched the replay and analyzed at 0.5x speed.

This is the exact moment when you press K1 but as you can see the mouse is just outside of the hitbox of the circle, meaning you are bound to get a sliderbreak.
EDIT: You pressed even earlier than that: http://puu.sh/ogEVq/f9a2e51f48.png

even when a note is hit PERFECTLY, meaning, cursor is dead on center of the slider note
This replay proves you otherwise.

I also watched the whole replay and you had 2 other sliderbreaks, both because there were stacked notes on the slider but you didn't press the buttons enough times (eg. triple+slider=4 presses but you only pressed 3 times).
