- hmm, some NC suggestions - In hard/insane/extra difficulties, it is usually recommended to start NCs every major white tick. So... start a NC at 00:44:129 (5) - 00:48:129 (7) - 00:52:129 (4) - etc. etc. I will check it but some places I want to keep for emphasis the rhythm

Insane is pretty good! I think the major problem in your extra/ insane is you are very erratic with your beat placement. Try to place your objects in such a way as to emphasize stronger ones (possibly by increasing the distance). Erratic placement can also make the map more difficulty/ annoying to play.
