Here's my mod as requested.

[General]
Timing and Offset: The offset of the song should be 385 since that's the time the song actually starts.
Kiai: The chorus is a good time to have Kiai time for a better feeling of the song, so consider adding that.
Snapping: There's a lot of unsnapped notes even with the current timing, so snap them back using 1/4 Beat Snap Divisor.

Be sure to apply the mod first before applying the timing/offset and snapping suggestions!

[]
[Normal]
Mod inside
00:06:298 (6298|0,6407|6) - Mistimed notes, move up to 00:06:407 - to be on time with the drum

00:17:904 (17904|6,18342|4) - Switch these notes around by column for piano pitch relevancy

00:33:232 (33232|2) - Move to 5, then 00:33:451 (33451|6,33670|5,33889|4,34108|3,34327|2) - rearrange this to 3,4,5,6,7 arrangement for base guitar pitch relevancy

00:51:626 to 01:19:218 - This part is much easier than the patterns earlier. Try to add more notes in this section to emphasize the atmosphere of this song.
For layering more notes in, I'd suggest adding in one more note for each snare that you hear. Example, 00:51:626 - this should have one more note to emphasize the snare. Same for 00:52:502 - , 00:53:378 - , 00:54:254 - , and so on and so forth.

01:04:546 (64546|6) - Move to 4 for easier playability

01:05:203 - Add a note here to accent the crash better

01:24:473 (84473|0,84692|2,84911|4,85130|6,85349|2) - Ctrl + H for guitar pitch

01:25:568 (85568|2) - Move to 4 for easier pattern execution.

[Hard]

- The LNs on this difficulty are highly similar to Insane, especially on the first half of the song. Try to rearrange some of them so each difficulty adds its own uniqueness to each other.
(Note: This suggestion appears in Insane. If you applied this suggestion for Hard, you don't need to apply this for Insane since each diff is unique to their own, and vice versa.)

00:10:240 (10240|1,10240|0,10458|6,10458|4) - Ctrl + H for drum pitch. The snares tend to be higher on pitch than the kick/base.

00:11:991 (11991|2) - Move to 5. Reason being is to lessen the tension on the left hand.

00:13:524 (13524|5) - Move to 5 for better pattern flow

00:20:969 (20969|6) - Same suggestion as above

00:28:196 (28196|0) - Move to 2, same reason as above

00:32:794 (32794|6,33013|2) - Switch these notes around by column for better playability

00:33:889 (33889|2,34108|1,34327|5) - Rearrange the note position to 5,6,7 respectively for base guitar pitch

00:40:896 (40896|6,41115|4,41334|5,41553|3,41772|4,41991|2,42429|1) - The current arrangement for these LNs are a bit random for the e-guitar, so try rearranging the notes around to follow the e-guitar's pitch.

00:44:838 (44838|2,45057|3,45276|4) - Ctrl + H, then move one column to the left for e-guitar pitch

00:46:152 (46152|4) - Move to 4 for pattern synergy

00:46:371 - Add a note here for note consistency, and for the snare

00:47:575 (47575|4) - Move to 4 for playability. The shield there will throw off players at this level.

01:03:451 (63451|6) - Move to 4 for drum pitch

01:08:269 (68269|5) - Move to 5 for better readability

01:13:524 (73524|5) - Move to 1 for hand balance

01:24:802 (84802|5) - Ghost note, remove (you didn't have this mapped on Insane)

- Same as Hard, the LNs are highly similar to Hard. Rearranging some of the LN patterns would add more variety to the mapset as a whole.

00:01:480 (1480|4) - Move to 2 so the 1/2 jack is on the left hand while the LN is on the right hand for better playability

00:03:013 (3013|2,3232|2) - Same idea as above, try to find a way to move the 1/2 jack to the right hand (preferably on column 5) so the pattern is a bit easier to do. Even with the slow BPM, this is pretty much more of a playability suggestion.

00:06:079 (6079|0,6079|6,6079|4,6079|2,6079|3) - 5-note chords is too strong for the drum snare and crash alone. 3-note chord or 4-note chord should be enough for that.

00:10:021 (10021|5,10240|4,10458|3,10677|2) - The piano's pitch is ascending but the LNs here are descending in pattern. Try to find a way to reverse the LN arrangement here for piano pitch relevancy.
Here's a pattern suggestion for this portion of the song. This one avoids some 1/2 shielding while keeping pitch relevancy in check.

00:14:181 - Add a note here for the hi-hat.

00:18:123 (18123|5,18342|4,18561|3) - Should be an LN trill to follow piano pitch relevancy here

00:21:845 (21845|6,21845|4) - Switch these around by column for guitar pitch

00:22:721 (22721|6) - Move to 4 for a better pattern flow

00:25:130 (25130|4) - Move to 6, then 00:25:349 (25349|5,25349|6) - move LN to 5 for pitch and move regular note to 7 for better synergy

00:31:480 (31480|0) - Move to 2 for better playability

00:40:896 (40896|6,41115|4,41334|5,41553|3,41772|4,41991|2,42429|1) - The current LN pattern seems a little random, so try to rearrange the notes around and arrange the LNs based on e-guitar pitch for the best effect.

00:44:838 (44838|0,45057|1,45276|2,45495|3) - These should be in a descending pattern to follow the electric guitar pitch here.

00:46:590 (46590|6) - I don't really see a reason why that minijack's there as it doesn't really fit with the music. Move to 5 to avoid mini-jack

01:00:276 (60276|3) - Move this note down to 01:00:057 - since the drum actually starts there

01:01:261 (61261|5) - Move to 2 to put in a little vocal pitch relevancy for the left hand

01:03:232 (63232|5,63232|6) - Remove one of the notes here, 4-note chords are a bit much to express the e-guitar and drum alone.

01:09:692 - Missing drum note here

01:18:561 - Maybe add a note here so this little section doesn't feel a bit empty? Same suggestion for 01:18:670 - .

01:23:159 (83159|1) - Move to 1 for better flow

01:24:364 - Missing snare note

01:24:692 (84692|6,84692|0) - Move to 2, 6 respectively for comfortability.

Well, the mapset itself looks good. Only major issue is that pitch relevancy for the electric guitar and other instruments weren't really followed on this map. The note technicality is okay, but needs a bit of tweaking, probably because the map currently has 1 mod applied. Look for more mods!

Hope this helps! Good luck.
