How do I get stacks into a pile where I can only see the top note and all of the others approach circles. Like this http://i.imgur.com/Bc1H8CM.png

I guess you were playing this map https://osu.ppy.sh/s/1785
The answer is; don't play this map!

The more serious answer is; play more!

You can figure out the timing of the circles by looking when each circle appears on the screen (or alternatively looking how compressed the approach circles are, but that doesn't work as well IMO), most of the time the circles on the stacks are either 1/4 or 1/2 notes. Having a skin with semi-transparent or transparent hitcircles makes it easier.

Thisis feature called Stack Leniency., which can be changed via editor (Song Setup -> Advanced -> Stack Leniency)

If you want it to be disabled like at this screen you should change Stack Leniency value to 0 via notepad (File -> open .osu in Notepad -> change in "StackLeniency=" line)

But be careful while using this - disabled stack leniency is very hard to handle properly while mapping, and it shouldn't be used very often - it usually better to keep it enabled (at least below 0.3-0.4) to keep map easier to read.

About how to play this - keep your eyes at approach circles!

I don't think he's asking how to play them. I think he simply wants to know how to make them.

In which case yeah, disable stack leniency. Wouldn't recommend using it though!
