Hold the pen horizontally and you should be fine.
It's only playable if you play really easy maps though since they don't require constant mashing.
The good thing with Taiko is that it really only require 2 buttons to "play" the game though the 2 additional keys are for patterns and generally to make it much easier to play in the long run.
So it's "possible" but uncomfortable/not good playing Taiko on a Tablet/Pen regardless of it being possible or not-
