for anyone curious, i got it by passing a map of Life Sux by Leah Kate with DT (7.21 stars, also didnt show me i got the medal when i did pass it so i played it 3 times, could that be a bug?).
id like to mention im pretty bad at being consistent at streaming, like flow aim, finger control (tapping fast, slow, speeding up, slowing down, basically EVERYTHINGGG), lots of notelocks, etc, and i also have a bad habit of (during jumpy parts of maps) moving my cursor over a circle but not clicking until my cursor is already off of it. either that or tapping too early for 100s or notelocks.
