idk if you're referring to mental disorders, but I used to have crazy rituals when I do everyday things, like washing hands, and often in repitition. I didn't really feel any guilt when I do it, I just have a feeling that I need to do it and I satifsy it by following what my mind wants. On the other hand, I actually feel guilty when I didn't perform them. It's like an itch that you don't scratch.

I eat when I'm nervous or anxious, if my tummy starts hurting then yes I feel guilty

I do the same but I think it has to do more with anxiety.
