Place a slider before a break. Add repeats to it until the break is too short and disappears. There will now be new combos added to several hitobjects between it and the previous break.
How many new combos are added - and whether this happens at all - seems to vary from map to map. If you're having trouble replicating I can find a post a few good examples.
