i would recommend the following:
1) Play different maps, usually if you play/focus the same maps too many times in a row, you might deprove
2) Focus on another skillset, there's a chance you are lacking experience in playing a certain map type. Your skillset can stop improving if it requires another skillset to be better.
E.g.
If you're focusing mainly on speed, at a certain point, the notes will be so dense that they will require you to have jack as a skillset too to maintain anchory patterns.

3) Try to change your perception of improving. You might be leaving too much to
random chance for "improvement".
E.g.
98.7% -> 99% has a possibility that you just got lucky at that point in time
95% -> 97% is almost definitely an improvement, not by chance.
this applies to lower accuracies too, if you're focusing on maps to 99 ~ 100, you will run into plays that are better and worse entirely by chance.
I personally try to focus on maps in the 88 - 95 range. That's the spot where I know I can read it, but just not fast enough yet.
4) Following the previous point. Knowing what you may be uncomfortable with is important
You can try to detect the following:
4.1) Sitting position, height, angle
4.2) Elbow & arm position, height, angle
4.3) Keybinds, too high or too low. Keyboard position too.
4.4) Syncing. Explained below
4.5) Scroll Speed & Hitposition. Explained below
4.6) Hydration, breaks, take breaks some times, even taking a shower or washing your hands every now and then has proven to help boost plays
4.7) Try out different skins, the biggest game changers for me would be to have a clean, non-obstructive skin. Some skins use arrows, bars, circles, etc. you can just try them out for fun some time to see if there's some breakthrough
Syncing
There's 2 ways of syncing
Visual SyncThis is where you entirely rely on how fast the note falls to sync to the map. You can sync this by just not listening to the music. Find out where you
consistently hit with the Hit Error Bar (Options -> Score Meter Type).
To sync this, adjust
Hit Position or
Scroll Speed.
Hits that are to the left is EARLY, right is LATE.
Early: Hit Position too high,
Increase HitPosition:XXX Value and or Decrease Scroll Speed
Late: Hit Position too low,
Decrease HitPosition:XXX Value and or Increase Scroll Speed
Audio SyncI usually sync this with my keyboard taps. So it's somewhat important that you can hear the taps.
There will always be a delay in the key tap compared to the game, I personally recommend the physical key tap sound because it's more realistic.
Listen to the keytap and sync with the song exactly. Try to find the perfect Global Offset (Options -> Global Offset) for yourself, I personally use -53ms.
Scroll Speed & Hit Position
Note:
skin.ini -> HitPosition
starts from the top of the screen. That means
500 is low,
250 is high.
Note 2: This is fixed scroll speed
I try to find a good combination of Scroll Speed and Hit Position for myself, I personally think it's good to have an intentional delay before I should hit the notes, so that I don't tense myself too much.
Personally, I increase speed and lower hitposition when:
1) I'm reading too fast
2) I'm hitting too early
3) I feel that there's too much delay
4) I feel like I'm predicting some notes
I lower speed and increase hitposition when:
1) Notes are too dense
2) I'm tensing too much
3) I just woke up
Note that you can just change the hitpos or speed, doesn't have to be both.
