There are currently medals for reaching rank 50,000; 10,000; 5000 and rank 1000 on the global leaderboards.
Since the community puts a lot of weight on reaching certain digit thresholds I suggest medals for rank
rank 100, 10 and 1.
Additionally one could follow the previous pattern and add the medals for rank 500, 100, 50, 10 and 1 (I think a medal for rank 5 is unnecessary).
Additionally, there could also be new medals like these based on country ranking instead, which would reset if a player changes the country.

Map leaderboards:
These medals would be achievable through gaining a certain rank on any leaderboard that belongs to a ranked map. These medals will only count for scores set 1 week after the map has been ranked. I think 1 week is good since this would prevent people getting these medals easily, especially since a lot of player submit D rank dtnf scores on a map as fast as possible and it also takes a week for a map to go from qualified to ranked so I thought why not take the same time (especially since 2 weeks sounds way too long).
I suggest a medal for rank 1, 50, 100 and 500 but maybe medals for rank 10 and 1000 as well.

Play count:
Add medals for 75,000 and 100,000 plays for those dedicated players. There could even be some for above 100,000 plays for those that are insanely dedicated to the game.

Total pp:
Medals that are based on a player's total pp. While I do think that a player's rank should only be determined by their top play, I don't see the current system based on the top 100 plays ever changing.
So why not give medals for certain thresholds. Sure, we already have medals for reaching top 50,000; top 10,000... but I think there should also be medals for other achievements, like gaining a certain amount of total pp. These could start at 100 or maybe 500 total pp up to 40,000 total pp (Mrekk currently has 30,000 total pp so why not make medals for up to 40,000).

Unique beatmaps played:
These medals are achievable through setting scores on unique beatmaps and go into the same category as
the completionist title. I suggest starting at 100, then 500, maybe have 750 in as well, then 1000... .

Star pass:
Due to the rising skill level of top players which has surpassed the realm of 10 stars, I think it makes only sense to prepare medals for those higher ranks as well. I suggest adding medals for 11 to 15 stars for now. Design wise, if you don't wish to cramp mpre stars into the medal, maybe color them pink in the order of which the normal star medals go. The pink star would count as 2 stars, so that when there is a medal with 9 white and 1 pink star, it would then be the 11 star medal. I chose pink since it represents osu! the best.
Disclaimer: The following is highly subjective to such a degree that I am not willing to go through the headache of discussing this. If this piece gets rejected then I will accept that without any debate at all.
I think that all star pass medals should only count for a maps star rating with nm. I believe that dt is not only broken in terms of pp but also in terms of star rating and there are undoubtedly maps which gain too much of a SR boost from hr as well. Therefore, if a map is 5 stars and one puts dt or hr or whatever on it, that it should still only count for a 5 star medal. Now, even if dt and other difficulty increasing mods do not have broken SR, this change would not be an issue since these people can just get that star pass with nm, since they are good enough to achive a pass on that star rating with a difficulty increasing mod and if those do not give too much of a SR boost, then the player will have no trouble getting the medal without those mods.

New rank ones: NO ones for less rank are stupid, as the amount of people that are gonna get it is stupidly low. It would make more sense to extend these the other way and add a #100'000 instead.

Map Leaderboards: These are kinda un-needed as #1 scores are already tracked somewhere else on profiles.

Play Count: Taiko already has a 30M hitcount medal that takes most people over 8 years to get, even taiko players think it's a bit dumb. I could imagine 100k plays working only if playcounts across modes are merged.

Total PP: This was brought up by devs before and the overwhelming answer was 'nah'

Unique beatmaps played: This is rewarded via the bonus pp system instead.

Star Pass: 10 looks nice and you'll get pitchforks if you try adding any more

This feels like turning medals into a stats tracker, which could probably be better done in other ways as this just 'bloats' up medals imo.

A top 100k rank medal makes sense, with the increase in playerbase the last few years making it more competitive to get.

Map leaderboard medals personally doesn't make sense for anything other than rank 1, because why should any other position on a map really matter, and there already is a medal for that so I don't think adding another one with the only difference being a 1 week wait to get it makes sense. Also, for the top 100, top 500 and 1000, a decent number of maps don't even have that many submitted plays after a month, let alone a week, so it wouldn't be much of a medal to get.

Playcount for 100k maybe but you'd have to add another medal for other gamemodes which would be harder to do.

Unique beatmaps played could be nice for say 5000 maps maybe. Most active players would have around a 1000 unique beatmaps played regardless just cus thats how much you need for max bonus pp, so I don't think medals for less than that would be a good idea.
