Another one I found underrated is Rec! from 2002. There was an animated adaptation made in 2006 I think.

For anime it would likely have to be hinako note (yes, the anime from the studio that made haitai nanafa).
As for manga, I guess exterminator. Pretty fun take on isekai imo
as for vn, sugar style is hella underrated. just because art style is not good, doesn't mean the rest of the game is bad.
