You should be able to just double click on it and osu! will extract it for you if it's a .osk file.
Otherwise, if you downloaded a .zip file or a .rar file, locate the Skins folder inside osu!'s main folder (installation location may vary but it's usually in [User]/AppData/Local) and extract the folder with the skin there.
