00:12:320 - Can't hear this, could still map 00:12:204 - as a slider that ends here so there isn't a random hole in the rhythm.
00:12:550 (1,2,3,4) - This whole part feels off, listen to the drums, for example.
00:14:166 - There's no beat here, make it a sliderend or similar instead.
^ 00:16:012 - No clear beat here either. You may say that it's following the guitar, but honestly I can't hear it. If there's an guitar sound, it's at 00:15:896 -

Any part like this, I think you should make into a sliderend instead on this difficulty. On previous ones you usually just had a slider running over it, but for this one you can fit in another circle where the sliderend would've been like this. It's a harder difficulty after all.

00:41:166 (1,2) - I don't think a jump will fit here, especially not such a strong one, because (1) and (2) are in the same measure and jumping on an offbeat can be a bit strange imo.

Are you sure the difficulty settings for precise are ok? OD 8 is a bit high for an insane imo. I'm sure OD 7 would suffice, especially since the circle size is so small. Could you imagine someone playing it with HR? xD
Combo colour 3 and 4 are very similar, so perhaps you could switch places with colours 3 and 4.
Sound files and background resolution are fine.
