Hmm... i dunno what you mean "how to get this working as a score bar on a skin" but if you download the file from the first page that's maybe the problem, so to solve this just open it with photoshop CC, edit the avatar frame & text, then save it to your skin folder but if you not @2x user just rezise it to 50%.

I would recommend to include a scorebar-marker on the templates aswell as the basic images to not mess up the positioning.

When I try to extract the PSD rar file it tells me the password is wrong. :\ Could you reupload it without an password?

Your PSD file's having some password issues. So, I gave up and just did this.
Also, The RAR file of the original SAO is screwed up on 7-Zip.

Hmm... the password not wrong at all, you can try to copy the password & ctrl+v it using winrar
