- You have to include hitcircle.png hitcircleoverlay.png reversearrow.png since you added the approachcircle.png
if you don't want to add them, delete the current approachcircle as well

01:19:676 (4) - nazi, x:412 y:200 is better imo (1.03x for both prev and next and also better gap between each other)

00:26:510 (6,1,2) - this spacing is confusing, the gap between 00:26:510 (6,1) - is almost like the 00:26:676 (1,2) -
it might be just me but this new combo doesn't help much , I suggest you put 00:27:010 (2) - under the 00:26:676 (1) - so that it will read easier then player just focuses only the approachcircle (doesn't need to focus on the spacing)
about 00:27:343 (3) - , I still think it's fine as is even the (2) is moved
01:09:343 (1) - as I mentioned in Easy

I'm not really sure about kiai on the middle of the song, they actually fit but it might be unrankable for some reason.
I suggest removing only

00:41:176 (5) - this slider on the 1st point and 3rd point (00:41:176 and 00:41:510 respectively) , I suggest you add a whistle with additions tab soft
so that it will fit the sound that you intended to follow on such 00:30:510 (5,6) - , I don't know how to call but I think you know

looked into the 1ms thing and unsnapped notes; hopefully it should be fixed now. (i made the offsets all the same through the .osu files)
