The quality of your mp3 file isn't good. You need to find one within the 128-192Kbps range.
According to #modhelp, background image has to be of 4:3 ratio (for example 800*600)
Preview point is not set. You can do it in Timing -> Set Current Position as Preview Point.
You need more than 1 diff to get this ranked. I suggest you to make a harder one.

After the bpm change you have to resnap all notes. You can do it in Timing -> Resnap All Notes.
