I've been playing osu on and off for 2+ years with a pen tablet and I've always had an issue with the thumb on my pen hand (right hand) aching about 2 minutes into playing a song.
I keep my arm and hand both as relaxed as possible and still get this aching sensation. It's really performance hindering.
Been holding my pen the same way for around 6 months
