Problem Details:

I have changed my username to Hikaru, but it still doesn't apply to either in-game or user page

This is the one should be change to Old-username : http://osu.ppy.sh/u/Hikaru

Send an e-mail to accounts@ppy.sh and you should get a quick response.

To be honest, I'm not really sure why your username hasn't changed yet. Anyways, start off by sending the e-mail!

What is more confusing is that the OP did send an email, and they probably applied the change again and the old name still remains.

I sent email last night, but still got nothing response :/ I just hope someone can fix this manually

I think there is a bug or glitch, I can still change my name, it indicate 'Is First Change: YES' even I changed it twice but it's just doesn't work

That's the first time I see something like this, probably it's just a bug that doesn't recognise your name change even if you try more times.

All you can do is asking accounts@ppy.sh to manually change your name, don't forget also to explain your issue exactly just like you did here.
